INVITATION FOR PRE-QUALIFICATION

 

1. Introduction

Chevron Nigeria Limited invites interested and experienced companies to apply for pre-qualification for the provision of Medical Services in Benin City, Sapele and Ughelli. The contract is proposed to commence in January 1, 2011 for two (2) years duration with one (1) year extension options.

 

2. Scope of work

Tender Specifications and Contract Condition. The successful Bidding Company (Contractor) shall provide primary and some secondary medical care services to company employees resident in Benin City, Sapele and Ughelli under the following conditions:

 

Services

CONTRACTOR shall attend to COMPANY’S employees, retirees and their dependants (“employees”) who possess COMPANY Medical Identity Cards at the Clinic. Generally, the CONTRACTOR may be requested to provide any type of general medical Services OTHER THAN those listed on the General Medical Exclusions list. COMPANY shall be under no obligations to reimburse CONTRACTOR for the provision of any Services provided to employees which fall under the exclusions listed.

 

CONTRACTOR warrants that it will perform the Services under this contract with the requisite medical skills and competence, administrative and Managerial skills and competent personnel necessary to carry out its obligations hereunder and that all Services shall be performed in a professional, diligent and workman-like manner.

 

In performance of the Services, CONTRACTOR shall abide by and be subject to the provisions of all applicable laws, ordinances, rules, regulations etc.

Stipulated by relevant professional bodies and governmental authorities and also Environmental Protection Practices Guidelines, Drug, Alcohol and Search Policy   

 

Contractor shall ensure that all its personnel are given the necessary training and continuing medical education (CME). Note that Company shall continuously verify that this is carried out.

 

PRE-QUALIFICATION REQUIREMENTS:

1.      Certificate of registration of company in Nigeria with Corporate Affairs commission together with Memorandum and Articles of Association and form CO7 showing list of directors and number of shares each has subscribed to.

2.      Evidence of joint venture agreement of memorandum of understanding in  case of existence of any partnership with a local or foreign firm and any sub-contractor intending to support the provision of any aspect of the service and clearly indicate roles and responsibilities of each party.

3.      Current Petroleum Inspectorate Commission (PIC) – (formerly called Department of Petroleum Resources) certificate or evidence of renewal; and current insurance policy.

4.      Evidence of registration or certification with any professional organization or association for this category of business.

5.      Current Tax Clearance Certificate for the past three years and VAT certificate

6.      Certified company audited account for the past three years including evidence of financial support from any reputable bank.

7.      Complete company and any partner or sub-contractor contact information stating key contact names: all office locations, mailing and email addresses with evidence of internet facility including telephone and fax numbers.

8.      Health Environment and Safety (HES) policies fatalities, motor-vehicle accidents, employee’s days away from work and total annual incidents in the past three years.

9.      Organizational chart with names, positions and qualification of key personnel Management, Financial, Quality Assurance and HES

10.  Provide personnel recruitment and sourcing strategy and capability to support the contract.

 

Provide details and evidence of similar job experiences within the last five years including names of client’s locations dates. Contract values and references on those jobs.

 

Nigerian Content Development Requirements:

Provide documentation and plans in areas not limited to No. 13 -25 below stating the required strategies to company with the Federal Government of Nigeria directives of October 13, 2006 to target achieving 45% and 70% Nigerian content Development (NCD) by year 2006 and 2010 respectively in terms of total monetary expenditure through the deliberate utilization of Nigerian human and material resources without sacrificing safety, health and environmental standards. Please note that non-submission of evidence demonstrating capability to comply fully with all the NCD requirements shall be considered a fatal flaw which shall result in disqualification from bidding for this service.

1.      Take note also that interested foreign companies must partner with Nigerian companies. Such companies must show evidence of joint venture agreement or memorandum of understanding duly signed by the CEO of both companies including evidence or plans of physical operational presence and set-up in Nigeria. In addition, they must confirm whether the contract will be awarded either solely through the Nigerian company or in partnership and that the work will be performed in-country.

2.      Provide a description of all works contractor proposes will be performed in Nigeria using Nigerian resources and what percentage of this work is the total work scope.

3.      Provide names and addresses of partnering Nigerian registered company and any other Nigerian company that will participate in this work including evidences of well equipped offices and workshop.

4.      Provide a brief outline of the scope of work, what items, materials and consumables would be procured in Nigeria and what percentage of this, is the total required materials and consumables.

5.      Provide evidence of what percentage of your key management positions is held by Nigerians and what percentage of the total work force are Nigerians.

6.      Provide what percentage of the work scope would be performed by Nigerians and what are your plans and target percent that the Nigerian workforce would have been increased over the next three years.

7.      Provide training, development and technology transfer program for Nigerian personnel and sub-contractors to be conducted in-country.

8.      Provide evidence and plans for capacity building and investment expansion programs or opportunities in human resources and infrastructure in Nigeria over the next three (3) years.

9.      Provide other information, innovation or plans for increasing local content development in the contract.

 

3.         SUBMISSION OF PRE-QUALIFICATION DOCUMENTS

SELECTION OF CONSULTANTS FOR BORE HOLE

EXPRESSION OF INTEREST

The Federal Government of Nigeria has received a credit from the international Development Association (IDA) awards the cost of Anambra State Health System Development Project -II, and it intends to apply part of the proceeds of this credit to selection of consultants for Bore holes (Feasibility studies, design and supervision) in selected Health Facilities in Anambra State.

Under the project, 2 bore hole shall be sunk in 2 Health Facilities in the State Namely, Mmiata PHC in Anambra East LGA and Ogwuaniocha PHC in Ogbaru LGA.

Terms of Reference

The consultant is required to carry out the following;

Carry out geological survey at the 2 designated facilities for the drilling of boreholes and the survey should contain information such as.

i. Nature of the rocks/underground geology;

ii. Hydrology /ground water potential of the areas in question.

iii. Viability of drilling boreholes in these areas;

iv. Advice on best method of drilling (e.g. Air percussion, mud (chemical), rotary drilling), etc.

v. Minimum and maximum depth of borehole per location

vi. Size or diameter of borehole most suitable for any given area;

vii. Where drilling is not feasible, should suggest any other alternative water scheme

viii. Undertake or provide guidance on bio-chemical analysis of the water and suggest the type of treatment or treatment plant required if any.

ix. Undertake the design of the most suitable water scheme, borehole, elevated tank & stanchion, water treatment plant and reticulation for the area and provide the specifications and BOQ. The drawings, original and as built shall be submitted to the project before final payment will be made to the consultant. The specifications and BOQ of the design to be submitted must be on computer diskette and hardcopy.

x. Supervision of works to completion and handover.

xi. Progressive report on stages of work during execution of contract.

TIME FRAME

The Geophysical study is expected to be completed and comprehensive report forwarded to the Project Manager on or before 6th November 2009.

REPORTING REQUIREMENTS:

The report shall be submitted in 4 hard copies and a diskette copy to the project management.

INVITATION:

The Anambra Health System Development Project now invites eligible Geographical Consulting Firm to indicate their interest in providing the services. Interested Consulting Firm must provide information indicating that they are qualified to perform the services (brochures, description of similar assignments, experience in similar conditions, availability of appropriate skills among staff, etc).

A Geographical Consulting Firm will be selected in accordance with the procedures set out in the World Bank Borrowers, January 1997 (revised September 1997 and September 1999) manual. 1.0 INTRODUCTION

The Nigerian National Petroleum Corporation (NNPC) intends to undertake the Rehabilitation of NNPC Industrial Clinic, Kaduna.

The Clinic is located within the NNPC Housing Estate situated on Kaduna Kachia Road Kaduna State.

This advert is to request interested and reputable Civil/Building Engineering Construction Companies to submit themselves for pre-qualification.

2.0 DESCRIPTION OF THE PROJECT (WORKS):

The works for the Rehabilitation of NNPC Industrial Clinic Kaduna shall include but not limited to:

2.1 Mobilization and demobilization of temporary facilities

2.2 Structural steelwork in roof members-

2.3 Roof covering with 0.7mm oven baked terracotta colour corrugated longspan aluminum roofing sheet fixed on steel purlines

2.4 Ancillary civil works as detailed in the bill of quantities

2.5 Internal and external painting of walls;

2.6 Reconnection of storm drain water piping;

2.7 Replacement of electrical fitting; and

2.8 All works shall be done in three phases as detailed in the drawings.

3.0 COLLECTION OF BID DOCUMENTS

All companies wishing to express their interest to bid for this project shall submit relevant documentation for pre-qualification as stipulated in 4.0 below. Only companies that are adjudged successful in the pre-qualification exercise shall be invited to collect tender documents upon payment of a non-refundable bid fee of N75, 000.00 (Seventy Five Thousand Naira Only) for the Rehabilitation of the NNPC Industrial Clinic, Kaduna.

All payments shall be in Bank Draft payable to the Nigerian National Petroleum Corporation. -

4.0 PRE-QUALIFICATION REQUIREMENTS

By this publication, the NNPC hereby invites competent, reputable and experienced civil/building engineering Contractors for pre-qualification to bid for the required services as described in 2.0. Interested companies shall submit pre-qualification documents detailing their Company’s capabilities, relevant experience and financial status as follows:

4.1 Full details of company profile with CAC certificate of Incorporation in Nigeria, Certified true copies of the memorandum and articles of association of the company and CAC form C07 (Particulars of Directors);

4. 2 Registration with Department of Petroleum Resources (DPR);

4.3 Registration with NNPC Headquarters or any of its subsidiaries in category C;

4.4 Proof of company’s financial capabilities, Including audited accounts for the last three (3) years along with corresponding tax clearance certificates, value added tax (VAT) registration certificate and evidence of past VAT remittance;

4.5 Organizational structure and available work force with CVs of key personnel evidencing adequate human capital for the requited services;

4.6 Evidence that the entity is registered with the relevant professional engineering bodies;

4.7 Details of foreign/local technical partners including joint venture/partnership inception date, scope and copy of partnership agreement;

4.8 Company’s CASHES and QA/QC policy;

4.9 Relevant and verifiable cognate experience with NNPC, or other organizations, on similar projects in the past five (5) years. (Please include job Completion Certificates, Year, Project Title, Client, Project Value and Duration, Client’s Contact Person, Phone number and e-mail address etc, for completed projects and award letters for on – going projects).

4.10 Provide list of construction equipment owned by the company;

4.11 Confirm willingness to commence with the execution on the basis of a Letter of Intent;

4.12 Nigerian Content: plan indicating utilization of Nigerian Materials, Goods, Services and Manpower, Established presence in Nigeria including investment(s) and plan for training/transfer of technology to Nigerians;

4.13 Details of safety records for accidents, injuries and damages for the past three (3) years and

4. 14 Any additional information that will enhance the potentials of the company.

5.0 SUBMISSION OF BID DOCUMENTS

The requested information and any supporting documents in respect of these requirements should be submitted in one original plus two hard copies and two electronic copies (CD ROM).
