INVITATION FOR PRE-QUALIFICATION
1. Introduction
Chevron Nigeria Limited invites interested and experienced companies to apply for pre-qualification for the provision of Medical Services in Benin City, Sapele and Ughelli. The contract is proposed to commence in January 1, 2011 for two (2) years duration with one (1) year extension options.
2. Scope of work
Tender Specifications and Contract Condition. The successful Bidding Company (Contractor) shall provide primary and some secondary medical care services to company employees resident in Benin City, Sapele and Ughelli under the following conditions:
Services
CONTRACTOR shall attend to COMPANY’S employees, retirees and their dependants (“employees”) who possess COMPANY Medical Identity Cards at the Clinic. Generally, the CONTRACTOR may be requested to provide any type of general medical Services OTHER THAN those listed on the General Medical Exclusions list. COMPANY shall be under no obligations to reimburse CONTRACTOR for the provision of any Services provided to employees which fall under the exclusions listed.
CONTRACTOR warrants that it will perform the Services under this contract with the requisite medical skills and competence, administrative and Managerial skills and competent personnel necessary to carry out its obligations hereunder and that all Services shall be performed in a professional, diligent and workman-like manner.
In performance of the Services, CONTRACTOR shall abide by and be subject to the provisions of all applicable laws, ordinances, rules, regulations etc.
Stipulated by relevant professional bodies and governmental authorities and also Environmental Protection Practices Guidelines, Drug, Alcohol and Search Policy
Contractor shall ensure that all its personnel are given the necessary training and continuing medical education (CME). Note that Company shall continuously verify that this is carried out.
PRE-QUALIFICATION REQUIREMENTS:
1. Certificate of registration of company in Nigeria with Corporate Affairs commission together with Memorandum and Articles of Association and form CO7 showing list of directors and number of shares each has subscribed to.
2. Evidence of joint venture agreement of memorandum of understanding in case of existence of any partnership with a local or foreign firm and any sub-contractor intending to support the provision of any aspect of the service and clearly indicate roles and responsibilities of each party.
3. Current Petroleum Inspectorate Commission (PIC) – (formerly called Department of Petroleum Resources) certificate or evidence of renewal; and current insurance policy.
4. Evidence of registration or certification with any professional organization or association for this category of business.
5. Current Tax Clearance Certificate for the past three years and VAT certificate
6. Certified company audited account for the past three years including evidence of financial support from any reputable bank.
7. Complete company and any partner or sub-contractor contact information stating key contact names: all office locations, mailing and email addresses with evidence of internet facility including telephone and fax numbers.
8. Health Environment and Safety (HES) policies fatalities, motor-vehicle accidents, employee’s days away from work and total annual incidents in the past three years.
9. Organizational chart with names, positions and qualification of key personnel Management, Financial, Quality Assurance and HES
10. Provide personnel recruitment and sourcing strategy and capability to support the contract.
Provide details and evidence of similar job experiences within the last five years including names of client’s locations dates. Contract values and references on those jobs.
Nigerian Content Development Requirements:
Provide documentation and plans in areas not limited to No. 13 -25 below stating the required strategies to company with the Federal Government of Nigeria directives of October 13, 2006 to target achieving 45% and 70% Nigerian content Development (NCD) by year 2006 and 2010 respectively in terms of total monetary expenditure through the deliberate utilization of Nigerian human and material resources without sacrificing safety, health and environmental standards. Please note that non-submission of evidence demonstrating capability to comply fully with all the NCD requirements shall be considered a fatal flaw which shall result in disqualification from bidding for this service.
1. Take note also that interested foreign companies must partner with Nigerian companies. Such companies must show evidence of joint venture agreement or memorandum of understanding duly signed by the CEO of both companies including evidence or plans of physical operational presence and set-up in Nigeria. In addition, they must confirm whether the contract will be awarded either solely through the Nigerian company or in partnership and that the work will be performed in-country.
2. Provide a description of all works contractor proposes will be performed in Nigeria using Nigerian resources and what percentage of this work is the total work scope.
3. Provide names and addresses of partnering Nigerian registered company and any other Nigerian company that will participate in this work including evidences of well equipped offices and workshop.
4. Provide a brief outline of the scope of work, what items, materials and consumables would be procured in Nigeria and what percentage of this, is the total required materials and consumables.
5. Provide evidence of what percentage of your key management positions is held by Nigerians and what percentage of the total work force are Nigerians.
6. Provide what percentage of the work scope would be performed by Nigerians and what are your plans and target percent that the Nigerian workforce would have been increased over the next three years.
7. Provide training, development and technology transfer program for Nigerian personnel and sub-contractors to be conducted in-country.
8. Provide evidence and plans for capacity building and investment expansion programs or opportunities in human resources and infrastructure in Nigeria over the next three (3) years.
9. Provide other information, innovation or plans for increasing local content development in the contract.

INVITATION FOR TENDER PRE-QUALIFICATION FOR CEMENTING PUMPING SERVICES FOR OML 130- EGINA FIELD DEVELOPMENT EGI/C054
INTRODUCTION:
Total Upstream Nigeria Limited (TUPNI), operator of the OML 130, invite interested and reputable Contractors with suitably qualified employees, equipment, premises and relevant experience to apply for pre-qualification for consideration to tender for the cementing and pumping services for the Deep offshore EGINA Field Development.
The Egina field is located offshore Nigeria on block OML130 at 200 km off Nigerian cost by 1550m of water depth. Egina is a new standalone deepwater development using two DP rigs starting during Q1 2012 with activity continuing to Q4 2016. The reservoir being highly faulted with a channels and lobes type arrangement, the well trajectories are complex with a high proportion of wells having horizontal drains. All wells comprise a sand control device (open hole gravel pack stand alone screen or expandable sand screens).
2. BRIEF DESCRIPTION OF THE WORK SCOPE:
Potential contractors must be able to provide for a duration of Twenty Four (24) months with a Twelve (12) month option to extend, starting Q1 2012, the equipment personnel and services required for the provision of Cementing, Pumping Services & Products on at least one MODUs as follow:
- Procurement of cementing and pumping unit with all the necessary piping panels
- Cables, trays, all hardware, in order to supply a fully operational package to perform the work.
- Primary cementing job of all casing (20”, 14” and 10”-3/4- casing)
- Light slurries and /or foam cementing (design, formulation and equipment)
- Foamed spacers
- Remedial cement squeezes and Cementing plugs (losses side track and abandonment) Pressure tests
- Leaks off tests
- Miscellaneous pumping duties off well bore fluids (mud, oil, water brine, and acid into the well, LCM pills etc.)
- Cement design lab tests (including compatibility tests) and cement placement modeling (simulation including ECD, temperature, casing centralization etc.)
- Supply of down-hole tools (packages cement retainers, bridging plugs)
On the Worksite:
- Make up and/or pump various types of fluids including but not limited to cement slurries spacers including foamed ones, washes, water, brines, mud or acid pills to comply with well programme or TUPNI’s specification and requirements.
- To perform low and high pressure pumping and/or testing.
- Management of stock of cementing products and consumables.
- Utilization, maintenance and/or repair of all equipment
- To co-ordinate sample taking and delivery of cement/blend. rig water and additives for lab testing
- To act on TUPNI’s behalf in the overall management and quality control of the entire cementing operation, fluids, spacer and cement slurries in conformity with well program or TUPNFs specifications. The contractor shall ensure that the equipment on board is
- 100% certified to the beginning of the project, and is in a good working order prior to each planned operation.
- Preparation of technical reports as required by TUPNI (cementing report).
- Provision of daily/operation reports.
Onshore
- Contractor will provide a technical engineer dedicated to TUPNI for the Egina Project. Contractor will provide technical assistance and engineering services through its base laboratory with lab equipment and technician.
- Implementation of the bulk plant facilities to comply with well programme or TUPNI’s specifications and requirements.
- Design of spacer, cement slurries and /or well fluids in conformity with well program or TUPNI’s specifications (lab testing, software design).
3. PRE-QUALIFICATION DOCUMENTS
3.1 GENERAL SUBMISSION
To qualify for consideration, Contractors are required to supply two (2) sets of the following documentation, with the specific chapters separated by dividers, in the same order as set out here below (a) to (i):
a) A cover letter summarizing the contents of the prequalification document.
b) Details of company’s Health, Safety and Environmental Policy and Programme together with HSE Management System. HSE considerations on site, and throughout, the requirement are part of Contractor’s responsibility and Contractor must have Proven methods for eliminating risks associated with its own scope of services and at interfaces with others. These methods must be clearly indicated.
c) Safety records detailed for accidents, injuries, damages and summarized for near-misses and LTI’s for the past three (3) years.
d) Copy of Department of Petroleum Resources (DPR) Certificate of Registration for years 2008 or 2009, or evidence of renewal, relevant to the scope of work herein described
e) Details of Contractor’s Community Affairs Policy and programme and records. Including copy certificates, awards and the like, for the past three (3) years
f) Quality durance System certified ISO 9000 or equivalent for operations and maintenance and relevant to the scope of work herein described.
g) Detail of relevant verifiable experience of similar scope of work as described herein that Contractor as undertaken in the last three (3) years inside and outside Nigeria
h) (i) Demonstration of Contractor’s ability to carry out the scope of work as described in paragraph 2 herein.
(i) Provide details of Contractor’s Yard, Quaysides, Workshops, Base Facilities, Engineering Offices, Offices, Warehouses, Factories, Equipment and the like required to carry out the scope of work as described herein.
(ii) Submit a resources capacity plan and forecast, covering years 2012 to 2016 demonstrating that at present Contractor has sufficient needs to carry out the scope of work as described herein,
i) Reference of three of Contractor’s major customers, where similar a scope of work has been completed, who can be contacted for information and reference.
j) Contractor’s audited accounts for the past three (3) years and, if relevant Nigerian tax clearance certificates.
k) All relevant information concerning contacts: representative’s name telephone number facsimile machine number and e-mail address.
l) Additional Technical Information Evidence of capabilities for post delivery service support.
3.2. NIGERIAN CONTENT
In line with the Federal Government of Nigeria Directives on Nigerian Content target of 70% by year end 2010 and TUPNI general contracting-out policy to maximize Nigerian content on the Egina Project, tenderers are to include in their bids a Nigerian Content Plan on how they shall meet this requirement.
(a) Evidence of Contractor’s registration in Nigeria (certificate of incorporation)
(b) Contractor’s general structure and organisation, including names and curriculum Vitae of key personnel, indicating Nigerian Nationals on the Contractor’s Board, and identifying relevant to the scope of work herein described, two Nigerian Director’s.
(c) In case of a joint venture or partnership with another contractor provide a copy of the agreement or memorandum of understanding together with a description of the interest of each of the participants and the role of each shall play in the carrying out of the scope of work; herein described.
(d) Describe the locations of Contractor’s Management Offices, Engineering Offices, Fabrication Workshop and/or Yards. Procurement Centre or Centre of Core Business activities in Nigeria (relevant to the scope of work described herein),

INTRODUCTION:
Addax Petroleum OPL 291 Limited (hereinafter referred to as “AP291L”) invites interested companies to pre-qualify for consideration in tendering for the provision of the Scope of work as State below, required for the OPL 291 work programme.
The reimbursement for the Scope of work performed shall be on a “call off” schedule rates and/or lump sum basis, as requested by AP291L.

NIGERIAN CONTENT:
In accordance with the Federal Government of Nigeria Directives on Nigerian Content target of 70% by end years 2010, Companies are to include in their submission their plan to comply with Nigerian Content Requirements by identifying the Nigerian Scope of work as a percentage of the total overall scope of work in terms of the monetary value that will be created in-country by the deliberate use of Nigerian resources (personnel, equipment and materials) on this project
Companies are to include the following information in their submissions:

(c) Nigerian content training and development plan
(d) Location of the project management and Procurement and activities in Nigeria.
Non Compliance with Nigerian Content Requirements is a “Fatal Flaw” and will be a ground for disqualification
PRE-QUALIFICATION CRITERIA
In order to be considered for participation in the tender process, AP291L requires that any interested company must comply fully with the current Nigerian Content Directives and in addition must be an indigenous Nigerian Company or an indigenous Nigeria Company having a genuine a joint venture with a foreign company with duly signed MOU by chief executive officers of the two companies.
Only companies able to document the required qualifying experience, resources and proven work record will be invited to competitively tender for this scope of work
Companies are required to submit a Pre-qualification Screening Questionnaire to be collected at the address below or requested via electronic mail at contracts.pq@addaxpetroleum.com. The information to be submission in response to the Questionnaire shall include in addition to other relevant information, the following:
- Company profile and organizational structure including Names and resumes of key personnel with telephone numbers and e-mail addresses
- Copies of Certificate of Incorporation and valid DPR registration specific to the work
- Evidence of similar work experience in the least five (5) years, with names of operators’ representatives that can be contacted to verify history.
- Company financial details (including banking details), audited accounts and Tax Clearance Certificates for the least three (3) years
- Documented evident of any significant relationships with supplying contractors and subcontractors together with details of any technical support agreement with foreign contractors:
- A verifiable programme to address Nigerian Content Federal Government of Nigeria’s Directives on Nigerian Content target of 70% by year end 2010 and as contained in NNPC Directives of 13th October 2009.
- Details of community relations policy and strategy which clearly addresses current community relation issues
- An overview of company Health Safety and Environmental Policy, including safety statistics for the least three (3) years
- Details of Quality Assurance/Quality Control (QA/QC) Policy and Quality Manual

3.0 PRE-QUALIFICATION REQUIREMENTS
The pre-qualification documents should include the following:
- Evidence of company’s incorporation with the Corporate Affairs Commission
- Memorandum and Articles of Association.
- Company Profile and names/CV. Of the Principal officers of the Company
- Company’s address/office location with telephone numbers.
- Three (3) years’ Tax Clearance Certificate (2006-2008)
- Evidence of verifiable experience on similar projects executed in the last three (3) years, giving names of clients, contract sum, and evidence of practical completion.
- Name(s) and address(es) or Banker(s) and reference letters from such banks
- Evidence of Financial capability to handle the project
- VAT Registration Certificate
4.0 COLLECTION OF TENDER DOCUMENTS
Since the pre-qualification and the full tender stages are to run concurrently, this advertisement serves as a notice for both the “PRE-QUALIFICATION” and the “PROPER TENDER”. The full tender documents are ready for collection from the office of the Academy’s Procurements Officer on payments of a non-refundable pre-qualification/tender fee of Twenty-Five Thousand Naira (N25, 000.00) only
5.0 SUBMISSION OF PRE-QUALIFICATION AND TENDER DOCUMENTS
Tenderers are required to submit separate sealed pre-qualification and full tender bids
The complete pre-qualification documents should be neatly bound and submitted separately in a sealed envelope boldly market at the top left hand corner “TENDER FOR …” The job for which the tender is submitted should be clearly indicated on the envelope (e.g. TENDER FOR THE CONSTRUCTION OF 1 NO. NEW 200 CAPACITY MALE CADETS’ HOSTEL”)
Both the pre-qualification and tender documents should reach the address in 7.0 below not later than 12 noon on November 27, 2009
6.0 IMPORTANT NOTE
a. Pre-qualification bids should be very clear about the area(s) of proven competence and interest
b. Failure to comply with any of the instructions above or to provide the information or any of the documents under section 3.0 may automatically disqualify a contractor.
c. Incorrect address or label on submissions and those received after the specified date shall be rejected
d. “Invitation for pre-qualification & Tendering” shall not be construed as a commitment on the part of the Maritime Academy of Nigeria to award any form of contract to any company and /or associated companies, subcontractors or agents, nor shall it entitle any company submitting documents to make any claim whatever and seek any indemnity from the Academy by virtue of such contractors having responded to our advertisement
e. All expenses incurred in preparing and submitting pre-qualification and tender documents shall be borne solely by the prospective contractors
f. The Academy reserves the right to consider pr not to consider any/all of the documents received
g. The Academy will deal only with authorized officers of the companies submitting documents not through individuals or agents
h. Only companies that have been successfully screened, pre-qualified and found competent to prove the service(s) will have their tenders publicly opened at 2.00pm on December 11, 2009 in the Admin Conference Room, Maritime Academy of Nigeria, College Road, Oron, Akwa Ibom State. Interested parties are invited to attend.
