REQUEST FOR EXPRESSION OF INTEREST
The federal Government of Nigeria has provided funds in the 2009 Appropriation for the Ministry of Health and the Ministry intends to apply part of the funds for payment of services including Laboratory Service Provision Assessment (LSPA the ENGAGEMENT OF A CONSULTANT FOR THE LABORATORY SERVICE PROVISION ASSESSMENT (LSPA) OF HEALTH FACILITIES IN NIGERIA.
BACKGROUND
MEDICAL Laboratory Services play a pivotal role in the provision of evidence based health care delivery in Nigeria, Medical laboratory services are provided by the government and private sector (individuals and non-governmental organizations (NGO) and other non profit organizations). Though services provided offer cover diagnostic, public health, teaching and research activities, there is paucity of systematic data on the capacity of laboratory services in terms of measurable support to clinical service delivery scale-up. In addition, previous contribution to health service provision assessment did not adequately capture the depth situational analysis of medical laboratory services in the country. To address this shortcoming the Federal Ministry of Health (FMOH) through the Hospital services department intends to conduct a laboratory service provision assessment (LSPA) of all cadres of public health facilities it Nigeria to provide an evidence based assessment of the infrastructure, staff capacities, diagnostic and prognostic capacity, as well as logistics and information technology compliance available to support clinical service delivery in Nigeria
JUSTIFICATION AND OBJECTIVES OF THE ASSIGNMENT
Presently, different categories of medical laboratory services are offered at the three tiers in the health care system, In general, minimal laboratory service is offered at the primary Health Care (PHCs) centres, while basic laboratory tests are offered by the model/comprehensive health centers, clinics, maternities, and nursing homes. At the general hospitals, private hospitals and private laboratories, middle level laboratory services are offered. Specilalist hospitals, Federal Medical centres and tertiary institutions including research institutes offer top level laboratory services. However, the current trends and state of knowledge/practice in the industry.
Additionally, optimal contribution of the laboratory sector to evidence-bases health care delivery is limited by the following factors;
Lack of basic components necessary for effective medical laboratory function and efficient service delivery. These include basic physical infrastructure, laboratory supplies and basic utilities like electricity and water.
Inadequacy of qualified laboratory personnel across the various levels of the health sector and the absence of an efficient capacity building system.
The lack of national standards for establishment of medical laboratories, an inefficient accreditation policy and the absence of internal and external quality control and assurance systems.
Inadequate local funding and insufficient political will
Poor data integrity and date disaggregation per state, geopolitical zone and facility type have significantly compromised effective planning and decision making
OBJECTIVE
The objective of the assessment is to identify existing gaps and strengths in the capacity of these facilities to provide the basic and comprehensive laboratory services (diagnosis, prognosis and surveillance) required to support clinical services rendered to the general population.
SCOPE OF WORK AND EXPECTED DELIVERABLES
The consultant is expected to:
- Visit all Federal Health Institutions (Teaching Hospitals, Federal Medical Centres, Orthopedic Hospitals, Psychiatric Hospitals, Specialist Hospitals, selected Primary Health Centres) in the country to collect laboratory information/data on the infrastructure, staff capacities, diagnostic and prognostic capacity, as well as logistics and information technology compliance, available to support clinical service delivery
- Establish the current state of the laboratory knowledge and practice in each facility
- Determine the clinical utility of laboratory services in each facility
- Measure the impact of funding to laboratory service in each facility
- Develop a real time laboratory Management Information Systems (LMIS) template to measure the clinical utility of laboratory services.
- Proffer solution(s) and give necessary advice on how to improve the clinical utility of laboratory services.
REPORTING AND APPROVAL PROCEDURES
The consultant shall be engaged for a period of three calendar months at the end of which a final report will be submitted to the federal Ministry of Health through the Deputy Director (Hospital services department) in both soft and hard copies
INSTITUTIONALARRANGEMENTS
- From the scope of work enumerated above data are expected from all facilities supported by FMOH
- Report is expected to be in both hard and soft copy
BASIC INSTITUTIONAL EXPERTISE REQUIRED
The consultant MUST demonstrate the following:
Knowledge of the Nigerian Health care System
Excellent working knowledge of routine and research Public health laboratory practice
Experience in health system template development by Epi Info, Excel software are will be an advantage
Good institutional memory in project/programme management
Evidence of previous nationwide health facility based surveys implemented in collaboration with FMOH.
A Consultant will be selected in accordance with the provisions of the 2007 Public Procurement Act
