PRE-QUALIFICATION NOTICE AND INVITATION TO TENDER

1. The Office of the Surveyor-General of the Federation, wishes to pre-qualify reputable and competent companies who may wish to apply for pre-qualification and submit tenders for the under-listed projects.

2. SCOPE OF WORK

(A) Production of Street Guide

· Lot 1- Street of Ibadan Capital of Oyo State

· Lot 2 – Street Guide of Kano Municipal- capital of Kano State

(B) African Geodetic Reference frame (AFREF)

· Lot 3- Procurement and Installation of one (1) No. GNSS/CORS equipment (Trimble Net 8) and accessories (IGS standard) with three (3) years guarantee/maintenance period with power surge for Gembu (Taraba State)

· Lot 4-Procuement and installation of one (1) GNSS/CORS equipment (Trimble Net 8) and accessories (IGS standard) with three (3) years guarantee/maintenance period with power surge for Kebbi (Kebbi State)

3. ELIGIBILITY FOR THE PRE-QUALIFICATION

To qualify for consideration, interested companies are required to supply underlisted documents

(i) Certificate of Incorporation with Corporate Affairs Commission

(ii) Tax Clearance Certificate

(iii) Details of company profile including personnel

(iv) Evidence of technical competence and previous experience in similar projects/jobs

(v) For survey jobs, evidence of registration with the Surveyors Council of Nigeria (SURCON)

(vi) Payment of non-refundable pre-qualification of N10 000 per lot

(vii) Evidence of attendance of AFREF Stakeholders Forum within our outside Nigeria will be of added advantage

(viii) Any other relevant information that may enhance the chances of the company

4. SUBMISSION OFTENDERS

The pre-qualification documents must be returned in sealed envelope marked at the top left hand corner “Pre-Qualification document” stating the job the company is applying for while the corresponding financial bid should also be another envelop, marked “financial bid”. The two submissions i.e “Pre-qualification documents” and “financial bid” should all be enclosed in a larger envelop which must be sealed and addressed to The Secretary, Ministerial Tenders Board, OSGOF. The sealed envelope is to be dropped by hand into Tenders Box of the OSGOF on or before 12 noon Monday, 18th Mar, 2009. Opening of only ore-qualification documents shall commence at 1:00pm on same day in the conference room of the OSGOZF. On the next day, on or before 19th May, 2009 at 1:00pm, the names of pre-qualified contractors will be announced and their financial bids will be opened. The financial bids submitted earlier by those that were pre-qualified will be returned to them unopened. Contractors or their representatives, NIS, NGOs and the general public are encouraged to attend, please.

REQUEST FOR EXPRESSION OF INTEREST FOR THE PROVISION

OF CATERING SERVICES

The Nigerian College of Aviation Technology, Zaria is an approved Aviation Training Institution located in Zaria, Kaduna State. The College Trains Pilots, Aircraft Maintenance Engineers, Aeronautical Telecommunication Engineers, Air Traffic Controllers and other specialists for the aviation industry.

The College has a cafeteria where it serves meals for an average of 350 students daily. Thus, the college wishes to engage the services of an independent caterer to run the College cafeteria for a period of two (2) years renewable based on satisfactory performance.

The College now wishes to request for expression of interest from suitably qualified caterers/catering companies with not less than five (5) years relevant catering services experience.

SCOPE OF WORK/SERVICES

The caterer/catering company will be required to provide the following services:

i) Procurement of raw materials and preparation of quality meals

ii) Maintenance of kitchen equipment and furniture and fittings

iii) Serving of three (3) meals per day (Break-Fast, Lunch and Dinner) plus snacks.

iv) Other catering services as may be required from time to time

ACCOMPANYING DOCUMENTS

All Expression of Interest shall include:

i) Detailed company profile including registered address, name, and functional telephone and tax Nos. and email address(es) of the company.

ii) Name, address (e), qualifications and experiences of key personnel. Attach copies of certificates

iii) Certificate of Incorporation, Memorandum and Articles of Association

iv) Veritable Evidence of provision of catering services within the last (5) years, (2004-2008) in similar institution/organization or restaurants. Attach copies of letters of offer.

v) Reference from a reputable bank stating the financial ability to carry out such services

vi) Audited company’s statement of accounts and tax clearance for at least three (3) years (2006 -2008)

vii) Evidence of access to equipment and resources required to be deployed for the services. Please provide list of catering equipment available.

The completed documents in hard copy should be enclosed in an envelope, sealed and marked “Request for Expression of Interest for the Provision of Catering Services” and addressed to:

2. The Federal Government of Nigeria has received additional Credit from the International Development Association towards the cost of Adamawa State Health Systems.

Development Project II and intends to apply part of the funds to cover eligible payments under contract for the Rehabilitation of the College of Nursing & Midwife Yola,

Renovation/Expansion of the Adamawa State Health Systems

Bidders may bid for any one or more lots and discounts offered for the award of combined lots will be considered in bid evaluation. Evaluation of bids shall be on lot by basis. Award of contract shall also be on lot by lot basis.

Development Project II Office, No.35A, Sir Kashim Ibrahim way, Yola for a non refundable fee of ten thousand Naira or its equivalent in a freely-convertible currency for the first set and five thousand Naira or its equivalent in a freely convertible currency for any additional sets. Interested bidders may obtain further information at the same address.

5. Bids shall be valid for a period of 90 days after Bid opening and must be accompanied by security of at least 3% of the bid price or its equivalent in a freely currency, and shall be delivered to Adamawa State Health Systems Development Project II Office, No.35A, Sir Kashim Ibrahim Way, Yola on or before 12.00 Noon, Wednesday 20th May, 2009, at which time they will be opened in the presence of the bidders or their representatives who wish to attend.

1.0 INTRODUCTION

The Federal Medical Centre, Yenagoa, hereby invites interested reputable contractors with the requisite experience to apply for Pre-qualification and Tender for the execution of 2009 Capital Projects in the hospital.

2.0 SCOPE OF WORK

3.0 PRE-QUALIFICATION REQUIREMENTS

Interested pre-qualification bidders are to submit the following

i) Name, address, telephone numbers of their company.

ii) Evidence of incorporation or business registration.

iii) Evidence of registration with the Federal Medical Centre, Yenagoa.

iv) Evidence of Tax Clearance Certificate for the last 3 years (old contractors).

vi) List of similar contracts executed in the past

vii) Evidence of financial capability and banking support.

viii) Warranty on services or product (where applicable)

ix) Authority or license to distribute product (where applicable)

x) Any other information that will enhance the application

Payment of non-refundable processing fees of N35,000.00 per lot is required.

Scoring format and eligibility

In order to qualify for consideration of the commercial bid, a candidate must score at least 70% on the pre-qualification criteria.

4.0 SUBMISSION OF TENDER DOCUMENTS

The pre-qualification documents must be returned in sealed envelope, marked at the top left hand corner “Pre-qualification document” stating the job the company is applying for, while the corresponded financial bid should also be in another envelope, marked “Financial BidThe two envelopes i.e “Pre-qualification document” and Financial document” and “Financial” should all be enclosed in a larger envelop which must be sealed and address to the Medical Director, FMC, Yenagoa on or before 12 noon Monday 11th May, 2009. Opening of only Pre-qualification documents shall commence at 1.00pm on same day in the Board Room. On the next day 12th May, 2009. At 2.00pm the names of pre-qualified contractors or there representatives NGOs and the general public are also invited to attend.

Please Note the following:

(i) The submission of pre-qualification and Tender documents does not mean that Federal medical Centre, Yenagoa is committed or obliged to award any form of contract to you.

(ii) Federal Medical Centre, Yenagoa on her appointed agents reserves the right to verify the claims by the companies; companies shall therefore include necessary authority and guarantee for such verifications.

(iii) In addition, this advertisement is not meant for contractors who abandon project in the past in other Institutions or suppliers that specialize in the slae of reconditioned, refurbished or “tokunbo” items.
