Under the supervision of the RN, the licensed vocational nurse is primarily responsible for assessing, implementing, and coordinating the individualized plan of care in accordance with physician’s orders for the elderly, chronically ill, and those with rehabilitative or palliative needs. Implements the plan of care in collaboration with residents, families, and other health team members to positively impact clinical outcomes. Performs designated tasks/duties including: providing information to residents/families and other health team members; participation in Interdisciplinary Team (IDT) rounds; re-assesses continued care plan within scope of practice; documents re-assessment and implementation of plan; consults with RN for modification of plan as needed. Responsible for monitoring the CNAs to ensure that care is provided with dignity, and to ensure a safe and comfortable environment. Understands and adheres to the California Licensed Vocational Scope of Practice and The American Nurses Association (ANA) Code of Ethics for Nurses. Performs other duties as assigned. Follows PPH rules, policies, procedures, applicable laws, and standards. Carries out the mission, vision, values, and quality commitment of PPH.

Job Requirements
Minimum Education:     As required by certification and/or licensure

Preferred Education:     Not Applicable

Minimum Experience:     0 - 12 months related experience

Preferred Experience:     1 - 2 years related experience
Knowledge of skilled nursing facility care

Required Certification:     American Heart Association recognized BLS - Healthcare Provider

Preferred Certification:     Not Applicable

Required License:     Current CA LVN license

Preferred License:     Not Applicable
